MSW Program Degree Verification

Once your degree is reflected on your official Texas State Transcript and if you are seeking degree verification or certification of your education from the School of Social Work, that is different than the University-level degree verification from the Registrar’s Office, please consider the following.

You will need to complete as much of the form as possible and include a detailed e-mail/letter using the template below.

First, Complete the Form

Complete the portion of the form that is supposed to be completed by the applicant (e.g., name, DOB, address). Do not include your social security number on the form, you can add that information once the form is returned to you.

Also, complete the following areas:
• Date of Graduation
• Dates of Attendance
• Degree Cohort (e.g., Regular/Foundation vs. Advanced; Campus vs. Online; FT vs. PT)
• Setting of Field Practicum/Internship and Site Name
• Number Internship Hours (Total)

Second, Create and Send an E-mail/Letter

My name is ________________. I graduated from Texas State's MSW program in [MONTH AND YEAR]. I am currently [CURRENT ROLE OR SITUATION] in the state of [NAME OF STATE]. I am working toward licensure in the state of [NAME]. The [STATE DEPARTMENT OF X] requires as part of the process for us to complete the [NAME OF FORM – e.g., “Approved Program Verification Form” or “Certification of Education Form”]. The form I need completed is attached to this email. I will need it by [DATE NEEDED].

I have completed the portion that is supposed to be completed by the applicant (e.g., name, DOB, SS#, address). I have also completed the following areas:
• Date of Graduation
• Dates of Attendance
• Degree Cohort (e.g., Regular/Foundation vs. Advanced; Campus vs. Online; FT vs. PT)
• Setting of Field Practicum/Internship and Site Name
• Number Internship Hours (Total)
